Please fill out this form to report any unsafe behavior or incidents on trips you have been on, either by a trip leader or general member. You may keep it anonymous but if possible leave your name and contact information so we can confirm details with you. You may remain anonymous outside of the Safety Committee who can review this form if you wish. The Safety Committee includes the President, Vice President, Secretary of Sports, Health & Safety Officer, and the Diversity & Community Outreach Officer, and they are the only ones who can read your response.
