Fill out this form if you attended a trip and felt as though there was a safety concern for you or another member of the trip.
The form asks if you would like to remain anonymous. We totally understand if you choose this option. If you would like to provide your name, this will only be used to contact you if we need more information. No one other than the President and Health and Safety will know who submitted the report.
